Introduction

Reproductive Health means total well-being in all aspects of reproduction - physical, emotional, behavioural, and social.

A person is considered reproductively healthy when:

  • Their reproductive organs function normally and are physically healthy.
  • They have normal emotional and behavioural interactions in all sex-related matters.
  • They maintain healthy social relationships related to reproduction.

Reproductive and Child Health (RCH) Programme

The RCH Programme covers a wider range of reproduction-related areas than the earlier Family Planning Programme. Its aim is to create awareness, provide facilities, and build a reproductively healthy society across India.

Goals:

  • Promote Awareness: Spread clear information about reproductive health and the prevention of sexually transmitted diseases (STDs/STIs).
  • Provide Medical Support: Ensure the public has access to the medical facilities and care needed to build a reproductively healthy society.
  • Introduce Sex Education: Implement structured sex education in schools to help clear up common myths and misconceptions among youth.
  • Lower Mortality Rates: Actively work to reduce both the Infant Mortality Rate (IMR) and the Maternal Mortality Rate (MMR).
  • Educate on Family Planning: Guide couples and young adults on birth control options, maternal care (pre- and post-natal), the importance of breastfeeding, and infertility treatments like ART.
  • Enforce Legal Marriage Ages: Uphold the statutory minimum age for marriage, which is strictly set at 18 years for females and 21 years for males.
  • Incentivize Small Families: Encourage population control by offering support and incentives to couples who maintain small families.
  • Ban Sex Determination: Strictly enforce the legal ban on amniocentesis for sex determination to prevent illegal practices like female foeticide.

Factors Indicating Improved Reproductive Health

The following are signs/indicators that reproductive health in a society is improving:

  • Better awareness about sex-related matters among people.
  • Increased medically assisted deliveries decreased maternal and infant mortality rates (MMR & IMR).
  • More couples with small families → family planning norms being followed.
  • Better detection and cure of STDs like HIV, gonorrhoea, syphilis, and hepatitis B.
  • Overall, increased medical facilities for all sex-related problems.
  • Better techniques and new strategies are being implemented over time.

Key Reproductive Health Problems in India

Problem Description Government Response
Overpopulation Population ~350 million at independence → 1.35 billion by 2020 Family planning, RCH Programme, incentives for small families
Female Foeticide Misuse of amniocentesis for sex determination, resulting in abortion of female foetus PNDT Act, 1994 - ban on sex determination
STDs / STIs HIV, Hepatitis B, gonorrhoea, and syphilis spreading through unsafe sexual practices Awareness campaigns, free STD testing, and sex education in schools
High MMR & IMR Maternal and infant deaths due to lack of proper medical care during and after delivery Medically assisted deliveries, post-natal care under RCH
Early Marriage Marriages before legal age leading to early pregnancies and health risks Statutory minimum age: Females 18 yrs, Males 21 yrs
Infertility Inability to conceive after 2 years of unprotected cohabitation ART centres; IVF, IUI, GIFT, ZIFT techniques available

Key Points: Reproductive Health

  • Reproductive health, as defined by WHO, refers to complete physical, emotional, behavioural, and social well-being in all aspects related to reproduction.
  • India’s Reproductive and Child Health (RCH) programmes aim to promote reproductive health through family planning, maternal and child care, awareness creation, and access to medical facilities.
  • Education and awareness, including sex education for adolescents, help prevent myths, sexually transmitted diseases (STDs), unsafe practices, and promote hygienic and responsible sexual behaviour.
  • Preventive measures such as menstrual hygiene, genital cleanliness, planned parenthood, immunisation, and statutory bans on sex determination are essential for a healthy society.
  • Sexually transmitted diseases like syphilis and gonorrhoea adversely affect reproductive health, highlighting the need for early diagnosis, treatment, and improved medical infrastructure.
