It typically arises when a pathogen is newly introduced or changes (or when host immunity falls), causing transmission to exceed the normal baseline; an outbreak confined to one area is an epidemic, whereas spread across countries/continents becomes a pandemic. Effective control relies on rapid surveillance and case detection, baseline-incidence monitoring, vaccination where available, infection prevention (hygiene, isolation, PPE), and targeted measures such as vector control or safe water and sanitation.
